German pharmacies face financial strain as costly drugs drain cash reserves

Pharmacies across Germany are struggling with tighter finances as cash reserves shrink. A growing reliance on advance payments for prescription reimbursements is becoming more common. Yet this short-term fix may not solve deeper financial challenges in the long run.

The financial pressure on pharmacies has grown in recent years. One key factor is the rise in high-cost medications, which now make up 40% of the sector's revenue despite accounting for less than 0.5% of all prescriptions dispensed. Sales of these expensive drugs have more than tripled since 2010, forcing pharmacies to cover large upfront costs before receiving reimbursements.

Several clearinghouses now offer advance financing options to help pharmacies manage cash flow. These early payouts can prevent immediate financial distress, particularly when dispensing costly treatments. However, the flexibility comes at a price—clearinghouses charge fees for accelerated payments.

While advance payments provide temporary relief, they are not a permanent solution. Over-reliance on them can create shortfalls at month-end if used for routine expenses. In some cases, excessive dependence on early payouts may even push pharmacies closer to insolvency.

Regulatory changes have not played a role in this shift. No updates to legal frameworks or pharmacy financing practices have been introduced in the last five years, leaving pharmacies to navigate these challenges without new support structures.

Advance financing helps pharmacies bridge gaps when dispensing high-cost drugs, but it does not address the root of the problem. With no recent regulatory adjustments, the sector must find ways to balance immediate cash flow needs against long-term stability. The trend towards expensive medications shows no signs of slowing, keeping financial strain on the industry.
